I do not have an HIN (Hospital Index Number) or an OPD card. How can I get one?
If you are an existing patient of our hospital, check your HIN number printed on the cover of your OPD book.
If you are a new patient or you have lost your OPD book, you can call us/ (contact us) between 9:00 AM – 4:00 PM in our front desk mobile 8974036093 and 7085040957 and get an HIN Number.
How can I book an appointment?
Once you pay through the Pay button on the right side of this page, we will schedule your appointment on receiving the payment confirmation and we will revert back to you.
Will I be able to get an instant appointment?
No. Currently for teleconsultation you are required to pay for an appointment at least a day ahead. Once we receive your payment confirmation then your appointment will be fixed for the next working day and our coordinator will contact you.
Can I avail teleconsultation without making any payment?
No. We will confirm your teleconsultation appointment only upon receiving confirmation of the payment.
How will I pay for the teleconsultation?
You can pay for the consultation from the Pay button on the right side of this page.
How much will it cost me for the teleconsultation?
We charge a minimal fee of Rs 500 only for a single teleconsultation session.
What details do I need to fill during the payment process?
You are required to fill all the mandatory details on the right side of this page before you can proceed for payment.
TERMS AND CONDITIONS.
- Reasonable efforts have been made to eliminate any confidentiality risk associated with the teleconsultation.
- All documented teleconsultation information will be archived for a period of 1 year for reference after which the information will not be eligible for any claims whatsoever.
- The Patient or the relatives shall be liable to legal actions under the IT Act if found recording and circulating the teleconsultation session in public domain.
- In the event of misconduct by the patient during the teleconsultation session, the consulting doctor has the right to discontinue the session immediately.
- Online payment made for the teleconsultation shall be refunded only if the hospital is not able to provide the service. Any lapse from the patient’s side shall not be eligible for refund.
- In case the mobile number provided by the patient during payment is unavailable for teleconsultation on the appointed day, the hospital will try to reconnect on the next working day. If still unavailable, then the appointment will be considered as availed with no refund.
- We will not be held responsible for any discrepancy in lab reports done outside CIHSR
- Patient is requested to send a response to the treatment within three days after the teleconsultation.
- Patient can avail only the assigned doctors for tele consultation.
- In case investigations are advised and the patient need to be reviewed again, patient need not pay for the same.
Disclaimer
Telemedicine has its own set of limitations for adequate examination. The diagnosis given by the doctor is only a Probable Diagnosis and the treatment given is Suggested. If a physical examination is critical for consultation, patient should meet the doctor in person.
